Target: Type 2 Diabetes℠

The Target: Type 2 DiabetesSM quality improvement program is built to support participants in improving care and reducing patient risk factors while honoring their effort and commitment to providing better care management to patients living with cardiovascular disease and type 2 diabetes.

By participating, organizations will have the opportunity to leverage initiative resources, educational webinars, and recognition opportunities.

Outpatient Practices

Outpatient facilities and care teams play an important role in patients’ type 2 diabetes management and can be recognized annually. View the full list of our 2022 nationally recognized organizations.

Recognition is based on submission of organizational information and aggregate measure data based on the previous calendar year. All data must be entered into our data submission tool before recognition closes at the end of May each year. Each Participant and Gold Award recipient will receive a certificate to display and access to digital promotional materials to honor achievements.

Registration is available year-round.

Inpatient Practices

Learn more about the important role that health care professionals in the hospital setting play in caring for type 2 diabetes patients admitted to the hospital with cardiovascular events.

Target: Type 2 Diabetes℠ provides healthcare professionals with resources and tools to address this urgent challenge as well as recognition in an honor roll award program for their treatment of heart failure and stroke patients with type 2 diabetes.
